In the Philippines, Chinese "surnames" are sometimes complete Chinese names, consisting of a surname and one or two given names. In your case, the actual surname is Tan (Chen, Chan)陳. As for Go and Nan it is hard to guess without the Chinese characters, for example, Go maybe "5" and Nan maybe "south".
