of Captain John J. Dix Company (H) of the Texas Frontier Regiment

of State of Texas born in [ Blank ] in the State of [ Blank ]

Aged [ Blank ] Years, [ Blank ] feet, [ Blank ] inches high, [ Blank ] Complexion [ Blank ]

eyes,[ Blank ] hair, and by profession a [ Blank ] was mustered into service

by Capt. John J. Dix at Concrete, Dewitt Co. on the 8 day of

March eighteen hundred and 62 to serve for 1

Year.. and is now entitled to a discharge by reason of Expiration of Service
